1c2d8ff92b avcodec/dsd_tablegen: Merge header into dsd.c 
Since b492fbcc6e, the DSD tables are
always initialized at runtime, so merge the dsd_tablegen.h header
into dsd.c.

4f5bd6177d avcodec/dsd: Make initializing DSD tables thread-safe 
This automatically makes the DSD formats as well as DST and WavPack
init-threadsafe.
