Some of these were made possible by moving several common macros to
libavutil/macros.h.
While just at it, also improve the other headers a bit.

These functions return an error typically when the key size is an
incorrect number. AVERROR(EINVAL) is more specific than -1.

It's been argued that the benefits of the current implementation far outweight
those of making the structs opaque.
This deprecation is not present in any release, so it can be safely removed.

For some of the code e.g. doing timing measurements there is no

This patch adds the possibility to calculate the DES-CBC-MAC of a
source buffer (i.e. the last block of the buffer encrypted in CBC
mode) without having to allocate a destination buffer that is as
long as the complete source buffer, but instead only 8 bytes
for the MAC.

Other parts of FFmpeg use NE (native endian) rather than ME (machine).
