To do this, allocate AVFilterGraphInternal jointly with AVFilterGraph
and rename it to FFFilterGraph in the process (similarly to
AVStream/FFStream).
The AVFilterGraphInternal* will be removed on the next major version
bump.

This commit moves the generic-layer stuff (that is not used
by filters) to a new header of its own, similarly to
5e7b5b0090 for libavcodec.
thread.h and link_internal.h are merged into this header.

Some of these were made possible by moving several common macros to
libavutil/macros.h.

These inclusions are not necessary, as cpu.h is already included
wherever it is needed (via direct inclusion or via the arch-specific
headers).
Also remove other unnecessary cpu.h inclusions from ordinary
non-headers.

This removes the XP compatibility code, and switches entirely to SWR
locks, which are available starting at Windows Vista.
This removes CRITICAL_SECTION use, which allows us to add
PTHREAD_MUTEX_INITIALIZER, which will be useful later.
Windows XP is hereby not a supported build target anymore. It was
decided in a project vote that this is OK.
If ret is NULL, a dummy common holder is created to hold *all* the
parallel function returns, which gets written concurrently. This commit
simplify the whole logic by simply not writing to that holder when not
set.
Sometimes, if pthread_create() failed, then pthread_cond_wait() could
accidentally be called in the worker threads after the uninit function
had already called pthread_cond_broadcast(), leading to a deadlock.
Don't call pthread_cond_wait() if c->done is set.
